29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  On or about June 10, 2014, at the above addressed jobsite, an employee was exposed to electrical hazards associated with using an electric power operated handheld circular saw not protected by a ground fault circuit interrupter.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On or about June 10, 2014, at the above addressed jobsite, employees performing commercial carpentry operations, sixteen (16) feet eleven (11) inches above the ground, were not protected from falling.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
