29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance:    Royal Engineered Composites: The employer is failing to ensure that Lockout or Tagout is performed only by the employee that is performing the servicing or maintenance. The employer is allowing maintenance employees to perform maintenance of equipment in the facility under a supervisor's lockout/ Tagout device only. The maintenance employee does not place their own LOTO device on the equipment being serviced as required by this standard.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    Bridgeport Machine in the Production Area: The employer is failing to protect employees from hazards created by the point of operation such as points of operation, rotating parts, flying chips and sparks in that one or more methods of machine guarding was not provided on a Bridgeport Series I Milling Machine Model# F-7347-00-828.
