29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    (a)  Vehicle Maintenance Building:  The left side mezzanine in the parts department had chains used in lieu of fixed guard rails where material was hoisted and the chains were not at the minimum allowed height to protect employees from falling approximately nine feet in that the middle of the top chain sagged down to twenty-nine inches and the mid chain sagged in the middle down to eighteen inches as observed on June 11, 2013.    (b)  Vehicle Maintenance Building:  The right side mezzanine in the parts department had chains used in lieu of fixed guard rails where material was hoisted and the chains were not at the minimum allowed height to protect employees from falling approximately nine feet in that the middle of the top chain sagged down to twenty-nine inches and the mid chain sagged in the middle down to fifteen inches as observed on June 11, 2013. 29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a noise monitoring program:    (a)  The employer had not conducted noise monitoring at this location to determine which employees should be in a hearing conservation program.  An employee (Track Maintenance) was exposed to continuous noise at 94.8 % of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 456 minute sampling period on July 16, 2014.  For a full eight hour work shift this exposure would be equivalent to an average sound level of 90.6 dBA.  Exposure calculations included a zero increment for the 24 minutes not sampled.    (b)  The employer had not conducted noise monitoring at this location to determine which employees should be in a hearing conservation program.  An employee (Track Maintenance) was exposed to continuous noise at 108.7 % of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 271 minute sampling period on July 16, 2014.  For a full eight hour work shift this exposure would be equivalent to an average sound level of 89.6 dBA.  Exposure calculations included a zero increment for the 209 minutes not sampled.
