29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) Production Area: While the employee was assigned to work with a mixture of 90% Nitric Acid and Acetic Anhydride, the employer did not assess heating effect of exothermic reaction between Acetic Anhydride and water containing in the Nitric Acid, which could lead to boiling and splash of the mixture, and the employer did not request from the employee to use proper PPE such as but not limited to, a faceshield and longs sleeve uniform, on or about 06/09/14.
