29 CFR 1910.106(e)(2)(iv)(d): Flammable or combustible liquids were not drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve:    a) Chemical Dispensing Area, Safer Textiles Processing, Inc. 1875 McCarter Highway, Newark, NJ 07104.    Workers dispensed Category 4 flammable liquids including, Starchem Starsurf WFF and Starchem Tanaterge SCP, found in 275 gallon plastic totes, via manually closing valves.     Conditions noted on or about 6/10/2014.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    a) Jet Dye House, Safer Textiles Processing, Inc.  1875 McCarter Highway, Newark, NJ 07104.    Employees operating Sample Jet No. 17 were exposed to an inadequately guarded belt and pulley located 62 inches above the work floor.       Condition observed on or about 6/10/2014.

29 CFR 1910.305(a)(2)(ix): All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard:     a) Jet Dye House Machines.  Safer Textiles Processing, Inc.  1875 McCarter Highway, Newark, NJ 07104.    Employees climbing a fixed ladder to access the work platform for Jet Dye Machine No. 11, 101 inches above the work floor, were exposed to unguarded fluorescent light bulbs which were within 2 feet of the top of the ladder.      Condition observed on or about 6/10/2014.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a) Jet Dye House Machines.  Safer Textiles Processing, Inc.  1875 McCarter Highway, Newark, NJ 07104.    Employees operating Jet Dye Machines were not trained in the hazards of the chemicals they work with to include but not limited to caustics such as light soda ash and flammable liquids such isopropanol found in StarChem Starsurf WFF and StarChem Tanaterge SCP.     Condition observed on or about 6/10/2014.
