29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: Location: 2604 Hwy 92 E. Plant City, FL 33566. a. Employee engaged in grinding and fitting of steel parts was exposed to continuous noise levels at 86.3% (equivalent to a TWA of 88.9 dBA) of the Action level (8-hour time weighted average sound level of 85 dBA) during a 464 minute sampling period on August 7th, 2014; exposure calculations included a zero increment for the 16 minutes not sampled.

29 CFR 1910.1000(c): An employee was exposed to crystalline silica quartz (respirable fraction) in excess of the 8-hour time weighted average limits of the calculated permissible exposure limit listed in Table Z-3: Location: 2604 Hwy 92 E. Plant City, FL 33566. a. an employee engaged in sand blasting of steel parts was exposed to respirable dust containing 45% crystalline silica (quartz) at a TWA level of 2.0 milligrams per cubic meter which represent 9.4 times the OSHA-PEL of 0.213 milligrams per cubic meter; this limit is established to prevent silicosis. The exposure level was derived from one sample collected over a 47-minute period; the remaining 433 minutes were considered zero exposure. Hazard observed on or about 07/02/2014.
