On or about June 11, 2014, an employee working for Building Solutions Group, Inc. was observed working inside the basket of a Dodge Truck Bucket 41' articulating boom aerial lift without a lanyard attached to the boom or basket an was exposed to a fall hazard greater than 25 feet to the ground below.
