29 CFR 1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  (a) Front of the Building - on or about May 30, 2014 - the worksite was not inspected by a competent person that was designated by the employer.  Observed hazards include improper ladder set-up, no fall protection from an elevated work area and an electrical hazard.

29 CFR 1926.404(b)(1)(i):  Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  (a) Front of the Building - on or about May 30, 2014 - electric tools were not protected by a Ground Fault Circuit Interrupter (GFCI).  The employer did not use an Assured Equipment Grounding Conductor Program.

Working elevation was approximately 12 feet, 1 inch.
