29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Building #5, North, core making department, Delta #64 carbon cutting vertical band saw: on or about June 10, 2014, an employee hand-feeding carbon roundstock bars for small parts cutting was exposed to contact with sharp edge amputation hazard where the unused portion of the blade was not guarded as required during operating cycle.  There is no abatement certification or documentation required for this item.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  Building #5, North, core making department, Redford core machine: on or about June 10, 2014, employees were exposed to caught-in between and crushed-under amputation hazards when producing cores and covers because the right side access gate's safety interlock switch was by-passed, rendering it ineffective as constructed and designed.  There is no abatement certification or documentation required for this item.

29 CFR 1910.304(f)(1)(viii): Circuit breakers used as switches in 120-volt and 277-volt, fluorescent lighting circuits were not listed and marked "SWD."  Building #5, I-T-E Pushmatic load centers and electri-centers circuit breakers for fluorescent overhead lighting fixtures: on June 10, 2014, employees were exposed to fire and burn hazards when utilizing circuit breakers not designed for routine use as an on-off control switches.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.303(h)(2)(i): Electrical installations in a vault, room, or closet or in an area surrounded by a wall, screen, or fence, access to which is controlled by lock and key or other approved means, are considered to be accessible to qualified persons only. The type of enclosure used in a given case shall be designed and constructed according to the hazards associated with the installation.  Building #5, 4160V electrical junction boxes: design and construction of fences in front of electrical installations did not prevent access of unqualified workers because they were less than 7 feet high, and not controlled by a lock.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
