29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked and/or decked between the front uprights and the guardrail supports: (a) 15 Cottage St., Easthampton, MA: On or about June 9, 2014 numerous areas of the welded tubular steel scaffold system where employees were walking on/working from to build a stair tower at this address were not fully planked or decked, exposing employees to falls of up to 50 ft. to the ground below. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used: (a) 15 Cottage St., Easthampton, MA: On or about June 9, 2014 employees were observed climbing out a 4th floor window to reach a scaffold platform that was 3+ ft. above the window sill they were climbing from and the employer had not provided any other acceptable access to the work location. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
