Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.    a) Building Exterior Front: Employees were working from a fabricated frame scaffold, six frames in height, at approximately 33 feet above the lower surface, without a standard guardrail system, on or about 6/12/14.    b) Building Exterior Front: An employee was working from the top of a plank that was positioned on the mid rail of a Marklift self-propelled scissor lift, raised approximately 33 feet above the lower surface, without a standard guardrail system, on or about 6/12/14.    c) Building Exterior Front: Employees were working from a Marklift self-propelled scissor lift, raised approximately 33 feet above the lower surface, without a standard guardrail system from the front section of the scaffold facing the building, on the section on or about 6/12/14.
