29 CFR 1910.1026(d)(2)(iv): The employer used the scheduled monitoring option, and monitoring revealed employee exposures to chromium (VI) to be above the permissible exposure limit; however, the employer did not perform periodic monitoring at least every three months: (a) Building 121 Bead Blaster Room: Personal air sampling of the cleanout of the bead blaster revealed an exposure over the permissible exposure limit for hexavalent chromium in 2011 and follow-up monitoring was not conducted each time thereafter when the bead blaster was cleaned on a periodic basis as determined on June 26, 2014. ABATEMENT VERIFICATION REQUIRED.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Fuel Cell Hanger: Employees were exposed to a fifteen foot fall hazard in that they were not required to wear the personal fall protection system that was in the work area while walking on top of the wings of a C-130 aircraft inside the hangar on August 12, 2014. ABATEMENT CERTIFICATION REQUIRED.
