29 CFR 1926.451(f)(4): Any part of a scaffold damaged or weakened was not immediately repaired or replaced, or removed from service until repaired: (a) 41 Worcester Rd., Webster, MA: On or about March 26, 2014, the scaffold system that employees were working from had numerous planks that exhibited damage, such as cracks and/or splitting, that weakened their structure. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.451(e)(1): Crossbraces were used as a means of access: (a) 41 Worcester Rd., Webster, MA: On or about March 26, 2014 an employee was observed climbing on the crossbraces of this tubular welded frame scaffold to access his work location on the scaffold. AA & D Masonry, LLC was previously cited for a repeat violation of this occupational safety and health standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 112526132, citation number 2, item number 1, and was affirmed as a final order on January 27, 2010 with respect to a workplace located on Route 1 in Scarborough, ME.
