Main St., East Brookfield, MA: On or about May 7, 2014, employees were observed roofing this residence more than 25 ft. above the ground below without fall protection. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: (a) 206 W. Brookfield St., East Brookfield, MA: On or about May 7, 2014, employees were observed using an aluminum extension ladder to access their roof top work location that only extended 2 ft. past the upper landing surface. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
