29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: This violation occurred on June 19, 2014, when the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels, at least annually after obtaining the baseline audiogram. a. An welder working at the burn table was exposed to continuous noise levels at 83.93% of the permissible 8-hour TWA (80 dBA). The equivalent dBA level of 83.93% is 88.7 dBA. Sampling was performed for 436 minutes. b. A mill operator was exposed to continuous noise levels at 86.67% of the permissible 8-hour TWA (80 dBA). The equivalent dBA level of 86.67% is 88.9 dBA. Sampling was performed for 466 minutes.
