29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a)    Fort Eustis Army Base Summerall Circle: On or about 06/12/14, employees in a 6'2" deep excavation replacing water main pipes were not provided with a means of egress, thereby exposing employee(s) to a cave-in hazard.   No abatement certification or documentation required.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.   a)    Fort Eustis Army Base, 2935 Summerall Circle: On or about 06/12/14, the spoil pile from the excavation was not kept from the edge of the excavation by at least two feet, thereby exposing employee(s) to a cave-in hazard.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  a)    Fort Eustis Army Base, 2935 Summerall Circle: On or about 06/12/14, an inspection of the excavation and its adjacent area was not conducted by a competent person by evidence of the spoil pile being less than two feet from the edge of the excavation, an inadequate protective system being utilized (improper benching for a granular cohesiveless soil), and no means of an emergency egress.            Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.652(b)(2): Maximum allowable slopes, and allowable configurations for sloping and benching systems, was not determined in accordance with the conditions and requirements set forth in Appendices A and B to this subpart.  a)    Fort Eustis Army Base, 2935 Summerall Circle: On or about 06/12/14, a 6'2" deep excavation was not protected from a potential cave-in, in that type "B" granular cohesionless soil was benched, thereby exposing employees to a cave-in hazard.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
