Elevation of the upper to the lower levels was approximately 4 feet, 9 inches.

The elevation of the upper floor to the lower floor was approximately 4 feet, 11 inches.

29 CFR 1910.23(e)(3)(v)(b): Railing construction type, size and/or arrangement did not meet the minimum requirement of strength to withstand at least 200 pounds top rail pressure: (a) Rail dock - on or about June 13, 2014 - the attachment hook for the chain gate that was used as a part of the guardrail system had significant damage, thereby weakening the integrity of this gate. The elevation of the upper floor to the lower floor was approximately 4 feet, 11 inches.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: (a) Cooler# 4 - on or about June 13, 2014 - 49 cases of apples were stacked in a manner that created a tipping/struck by hazard. The top of this stack was leaning forward. The cases were not banded or stretch-wrapped to prevent the cases from falling off the rack. The apple cases weighed approximately 40 pounds each. The elevation of the rack that the apples were stored on was approximately 9 feet, 5 inches from the concrete floor. (b) Cooler# 9 - on or about June 13, 2014 - 60 cases of pears were stacked in a manner that created a tipping/struck by hazard. The top of this stack was leaning forward. The cases were not banded or stretch-wrapped to prevent the cases from falling off the rack. The pear cases weighed approximately 18 pounds each. The elevation of the rack that the pears were stored on was approximately 9 feet, five inches from the concrete floor.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures: (a) Sanitation Closet - on or about June 13, 2014 - employees were exposed to an electrical hazard while using the slop sink to fill water into the mop bucket, and empty the mop bucket water into the sink. The electrical service panel is located approximately 8 inches to the right of this sink.
