Cross braces were used as a means of access: On or about June 12, 2014 at a job site located at 4080 Embassy Parkway, Fairlawn, OH, during construction activities including but not limited to masonry work from an elevated platform, the employer did not ensure the subcontractor employees working from an elevated platform were provided proper access to different levels.
