29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.    Location: 147-155 Main St. Ossining,  NY 10562    a)  Employees were mixing, pouring and applying Portland cement without the use of  safety goggles; on or about 6/10/14.       NOTE: The employer is required to submit abatement certification for this item in   accordance with 29 CFR 1903.19.

29 CFR 1926.50(g): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    Location: 33 Ashburton Ave.    a)  Employees were mixing, pouring and applying Portland cement and using Vana-Stop New Masonry Detergent Cleaner ( Hydrochloric acid) to clean bricks without the use of a quick drenching facility; on or about; 6/10/14.    The Dani's Home Builders was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.50(g) which was contained in OSHA inspection number 314978701, citation number 1, item number 3  and was affirmed as a final order on 8/10/11, with respect to a workplace located at Mount Vernon High School 100 California Rd. Mount Vernon, NY 10552.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (Construction Reference 1926.103)      Location:  147-155 Main Street, Ossining, NY     a)  Employees  are required by employer to wear respiratory protection such as MSA half-face negative pressure respirators when pouring, mixing and applying Portland cement. The employer did not implement a written respiratory protection program; on or about 6/10/14.    The Dani's Home Builders LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c)(1) which was contained in OSHA inspection number 314980467, citation number 1, item number 1A  and was affirmed as a final order on xxx, with respect to a workplace located at 94 N Mail St. Spring Valley, NY 10977
