29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: The employer does not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before an employee performs service or maintenance work on a motorized vehicle that the vehicle is isolated from the energy source and rendered inoperative. This violation occurred on or about March 8, 2014, in the workshop; where the employer did not ensure that before an employee replaced the drive shaft to a 1992 Chevrolet Blazer that the vehicle was parked on a level surface, the parking brake was securely applied and the wheels were chocked or blocked to prevent the vehicle from moving.

29 CFR 1904.39(a): Within eight (8) hours after the death of any employee from a work-related incident, the employer did not orally report the fatality by telephone or in person to the Area Office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor, that was nearest to the site of the incident, or by calling the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742): Following the work-related death of an employee that occurred on or about March 8, 2014, the employer did not report the incident of OSHA within eight (8) hours after the death.
