29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): Construction reference: 1926.602(d):  On and about 6/13/14, employees operating a rough terrain forklift demonstrated their lack of training and knowledge of forklifts when they were using the forklift to hoist personnel on an unapproved platform and operated the lift controls while standing on the ground outside of the cab.
