The employer does not ensure that all parts and appurtenances necessary for the safe and effective utilization of work platforms and scaffolds are integral parts of their design. a. This violation was observed on or about June 11, 2014 at Pep Boys #209 in the rotor aisle where employees were exposed to fall hazards when using a 5 step caster-type mobile work platform whose feet had missing or damaged slip-resistant crutch tips. b. This violation was observed on or about June 11, 2014 at Pep Boys #209 in the gasket aisle where employees were exposed to fall hazards when using a 5 step locking-type mobile work platform whose feet had missing or damaged slip-resistant crutch tips. c. This violation was observed on or about June 11, 2014 at Pep Boys #209 in the brake pad aisle where employees were exposed to fall hazards when using a 5 step locking-type mobile work platform whose feet had missing or damaged slip-resistant crutch tips. d. This violation was observed on or about June 11, 2014 at Pep Boys #209 at the end caps of the fuel pump aisle where employees were exposed to fall hazards when using a 5 step Locking-type mobile work platform whose feet had missing or damaged slip-resistant crutch tips. e. This violation occurred on or about June 10, 2014 at Pep Boys #209 where the employer failed to report to OSHA the work-related death of an employee who fell from a portable work platform. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the employer reports to OSHA work related deaths or in-patients hospitalizations of three of more employees within eight hours of learning of these events.
