29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):   On or about June 13, 2014, an employee was observed dropping roofing material over the edge of a residential roof into a trailer located on the ground that was greater than 20 feet exposing employees to a struck by hazard.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   On or about June 13, 2014, employees were using an extension ladder to gain access to the roof to perform roofing work on a residential structure without the ladder side rails extending three feet above the upper landing surface.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  The employer does not require the use of any form of fall protection during roofing work. This violation was most recently observed at 730 Northland Blvd, Cincinnati, OH 45240, where three employees were not protected from fall hazards of seventeen feet two inches to the ground.   To abate this violation, the employer must ensure that its employees performing roofing work are protected from falls, and that no employee is allowed onto a roof more than six feet high without adequate fall protection.  Richard Snader Installation Partners was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13) which contained in OSHA inspection numbers:  1) 306898198 Serious Citation 1, Item 2, Issued on 08/08/2003 2) 309962009 Repeat Citation 2, Item 2, Issued on 05/12/2006 3) 310515861 Repeat Citation 2, Item 2, Issued on 11/02/2006 4) 554638      Serious Citation 1, Item 1, Issued on 12/13/2012         Pursuant to 29 C.F.R 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including a description of how these steps protect its employees from the fall hazards.
