29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about June 12, 2014, during excavation activities, the employer did not ensure a means of egress was provided to employees working in a trench that ranged in depth of 5.5-6.2 feet deep. The trench was entered by employees on a worksite located at 12004 Chippewa Road, Brecksville, OH. J&J Enterprise Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 471519, citation number 1, item number 2 and was affirmed as a final order on September 25, 2012, with respect to a workplace located at 5771 Walling Road, Broadview Heights, OH.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. On or about June 12, 2014, during excavation activities, the employer failed to ensure adequate protection for an employee working in a trench that was approximately 5.5-6.2 feet deep by placing the spoils piles at least two feet back from the edge of the trench to prevent materials or equipment from falling or rolling into excavation on to the employee. This was done with respect to a worksite located at 12004 Chippewa Road, Brecksville, OH. J&J Enterprise Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(j)(2), which was contained in OSHA inspection number 471519, citation number 1, item number 3 and was affirmed as a final order on September 25, 2012, with respect to a workplace located at 5771 Walling Road, Broadview Heights, OH.
