29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about June 26, 2014 and prior thereto, the employer had not developed and utilized machine specific lockout tagout procedures for the Kardex Remstar Shuttle CP C2K located in Sort and Palletize. Employees performing maintenance and servicing work on the shuttle were exposed to the hazard of being struck by/caught in the equipment from the equipment not being blocked.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): On or about June 13, 2014 and prior thereto, employees performed electrical work on deenergized electrical equipment without locking out the energy source, exposing employees to the hazard of contacting live electrical parts and arc flash hazards in the following instances: a. rewiring a 110 volt AC foot switch b. changing fuse for a tire driver c. changing fuse for sort and palletize conveyor d. changing fuse for drive pullout conveyor at #3 Batch off e. running motor leads
