29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 321 SUMMIT STREET, WEST READING PA, ROOFTOP - Employees performed roofing work at heights ranging from approximately eleven (11) to twenty-seven (27) feet, and the employees were not protected by fall protection, thereby exposing employees to serious injuries, on or about June 4, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest systems, guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: (a) 321 Summit Street, West Reading PA, ROOFTOP - Employees performed roofing work at heights ranging from approximately twelve (12) to twenty-seven (27) feet, and the employees had not received adequate training on the use of the fall protection systems they were using, thereby exposing employees to serious injuries, on or about June 4, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
