29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about June 13, 2014, on the north side of the residence the employees used a 16-foot Werner aluminum extension ladder for access to and from the roof where the side rails did not extend 3 feet above the roof surface, being exposed to an 8-foot 4-inch fall hazard.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): a. On or about June 13, 2014, on the north side of the residence the employees used a 16-foot Werner aluminum extension ladder for access to and from the roof where the horizontal distance from the top support to the foot of the ladder exceeded one-quarter of the working length, exposing employees to an 8-foot 4-inch fall hazard.
