29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards: a) In Production department - The employer failed to ensure employees were wearing foot protection when they were operating the Eddy Current machine. Employees were loading and unloading scrap metals and operating the forklift. Employees were exposed to foot injuries due to falling or rolling objects.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead: a) In Break Room - The employer failed to ensure all surfaces were maintained free of lead accumulation. The by establishing a housekeeping procedure that included the cleaning of all rooms and areas to maintain all surfaces free of lead accumulation. Wipe sampling done on June 30, 2014, taken from a shelf where employees place their lunch bags, showed an accumulation of lead at 13.54 micrograms.
