29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Parking lot of La Quinta: During the operation of an aerial lift an employee was wearing a personal fall arrest harness but was not secured via a lanyard to the lift basket exposing the worker to ejection hazards. Condition noted on or about 06/14/2014.
