Jobsite - On or about June 16, 2014  at 98 Waverly Circle, Watervliet NY 12189 employees were exposed to an electric shock hazard because of a frayed cord to a portable radio.    c.  Jobsite - On or about June 16, 2014  at 98 Waverly Circle, Watervliet NY 12189 employees were exposed to an electric shock hazard because of a frayed cord to a table saw.

29 CFR 1926.502(k)(3): A copy of the fall protection plan with all approved changes shall be maintained at the job site.    a)  On or about May 17, 2014 at 98 Waverly Circle, Watervliet, NY 12189 in the North Ridge Hollow development construction jobsite the fall protection plan was requested but was not available on the jobsite.  Employees were exposed to a fall hazard without the fall protection plan on site to follow.
