OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from fire and burn hazards: (a) Facility, 1004 Industrial Boulevard, Area of Casting Die Room and Too Room Storage Area, Loyalhanna, PA: On or about 06/16/14 the 4" high pressure natural gas shut-off valve used to supply fuel to the furnaces was blocked with equipment and materials and not immediately accessible exposing employees to potential fire, explosion and burn hazards.

Several employees who operate the powered industrial trucks had last been evaluated in 2008, exceeding the 3 year requirement.
