29 CFR 1910.67(c)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket when working from an aerial lift:   a)    Clock tower, 931 Centennial Blvd, Voorhees - Employees without required fall restrain or  fall arrest equipment were working approximately 35 feet above ground level, from a Genie, Model S-25 Aerial Lift, while caulking the trim work around the clock face, on or about 06/10/14.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
