29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:      a) On or about June 13, 2014 and at times prior, an employee was exposed to flying particles or debris while using a bench grinder. The bench grinder was missing the adjustable tongue guard.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employees ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:      a) On or about June 13, 2014 and at times prior, employees were exposed to health-related injuries while using a tight-fitting face piece respirator. The employees were not provided with a medical evaluation prior to using a respirator.    The Euro Collision Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(e)(1), which was contained in OSHA inspection number 62929, citation number 1, item number 3(b) and was affirmed as a final order on 10/25/2011, with respect to a workplace located at 8536 S. Colorado Blvd., Littleton, CO 80126.

The employees were not provided with a fit test prior to the initial use of the respirators.   The Euro Collision Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 62929, citation number 1, item number 3(c) and was affirmed as a final order on 10/25/2011, with respect to a workplace located at 8536 S.
