29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. (a) 121 Liberty Street Brockton, MA 02301: On or about June 9, 2014 and days preceding and after, the employer did not record the following workplace injuries or illness on the OSHA Form 300 or equivalent for the calendar year 2014: (1) Case 1: On or about March 28, 2014, a temporary worker on the production floor suffered a laceration to their finger while opening bread resulting in an unknown number of lost work days; (2) Case 2: On or about April 4, 2014, a temporary worker on the production floor suffered a slip and fall injuring their hand resulting in 7 lost work days; (3) Case 3: On or about April 24, 2014, a temporary worker on the production floor suffered a laceration to his thumb while sharpening a knife resulting in 10 lost work days. Greencore OARS was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 315140004, citation number 2, item number 4 and was affirmed as a final order on June 1, 2011, with respect to a workplace located at 121 Liberty Street Brockton, MA 02301.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Preparation area: Grote Slicer, Model 613 US 2, Serial 1154604, had an inoperable safety interlock due to the screws that attach the safety interlock to the door were missing exposing employees to amputation and caught in hazards from the slicer blade. Greencore OARS was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.212(a)(3)(ii), which was contained in OSHA inspection number 315140004, citation number 1, item number 5 and was affirmed as a final order on June 1, 2011, with respect to a workplace located at 121 Liberty Street, Brockton, MA 02301.
