29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support: Employees used an extension ladder to access the roof of the apartment building at this site. The ladder did not extend at least 3 feet above the roof eave and the ladder was not tied or otherwise secured to a rigid support.
