29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about 16 June 2014, at the above addressed jobsite, employees were utilizing a table saw to cut trim to place inside the building under construction without utilizing eye protection, exposing the employees to the hazard of being struck by flying objects.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about 16 June 2014, on the above addressed jobsite, employees were utilizing a table saw that was powered by a yellow 120V extension cord that had a damaged strain relief, exposing the employees to an electrical shock hazard.
