29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.  a) Northeast corner of the residence, 875 Exeter Avenue, Exeter, PA - Employees mounted and dismounted onto the T-Shaped Main Roof from a Louisville L-3022 32 FT Type IA Fiberglass D-Rung extension ladder that was located within three-foot (3') of the live electric power supply line (240 V) for the residence. Employees were exposed to contact with an electrical current hazard, as observed on or about June 17, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) Garage Roof, 875 Exeter Avenue, Exeter, PA - Employees engaged in repairing and shingling the low sloped garage roof (3:12 pitch) of a residence were exposed to a fall hazard that ranged from twelve-foot, eleven-inches (12'11") to eight-foot, four-inches (8'4") above ground level. The employer did not ensure that fall protection systems were utilized to protect the employees from falling, as observed on or about June 17, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

The employer did not ensure that fall protection systems were utilized to protect the employees from falling, as observed on or about June 17, 2014.  b) Front Porch Roof, 875 Exeter Avenue, Exeter, PA - Employees engaged in repairing and shingling the low sloped front porch roof (2:12 pitch) of a residence were exposed to a fall hazard that ranged from fourteen-foot, two-inches (14'2") to twelve-foot, three-inches (12'3") above ground level. The employer did not ensure that fall protection systems were utilized to protect the employees from falling, as observed on or about June 18, 2014.In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a) Northeast corner of the residence, 875 Exeter Avenue, Exeter, PA - Employees mounted and dismounted onto a roof from a Louisville L-3022 32 Ft Type IA Fiberglass D-Rung extension ladder whose side rails extended approximately eighteen-inches (18") above the main roof eave and which was not secured to the upper landing surface. The employees were exposed to a fall hazard of twenty-two-foot, seven-inches (22'7"), as observed on or about June 17, 2014.  b) Southeast corner of the front porch, 875 Exeter Avenue, Exeter, PA - Employees mounted and dismounted onto a roof from a Werner D1120-2 20 Ft Type III Aluminum D-Rung extension ladder whose side rails extended approximately eighteen-inches (18") above the front porch roof eave and which was not secured to the upper landing surface. The employees were exposed to a fall hazard of twelve-foot, three-inches (12'3"), on or about June 17, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

The employees were exposed to a fall hazard of twenty five-foot, two-inches (25'2") and a struck-by hazard, as observed on or about June 17, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
