29 CFR 1910.1030(f)(3)(i): Following a report of a potential exposure incident, the employer did not document the routes of exposure, and the circumstances under which the exposure incident occurred. a) In the Medical Laboratory, On or about June 2, 2014 - A damaged centrifuge splashed blood or OPIM in an area in which employees were potentially exposed, and the employer did not appropriately document the circumstances under which the potential exposure incident occurred. Employees were exposed to a blood splash from a damaged centrifuge located in the immediate area. Employees were not protected from potential mucus membrane exposure during the blood splash. Employees were not provided an immediate confidential medical evaluation and follow-up related to the exposure incident to include documentation of circumstances, identification of source patient, collection of employees blood, and counseling once reported to a management official.
