29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about June 17, 2014, employees were not protected from falling over the edge of the bridge being dismantled on State Route 176, over the Interstate highway 271, in Richfield, OH. Employees were tasked with cutting steel bars as concrete portions were removed. Guardrail systems, personal fall arrest systems, and other fall protection devices were not in place. The distance to the level below was greater than 16 feet. b) On or about June 17, 2014, employees were not protected from falling over the sides of a ramp on the northern end of the bridge used by employees for access to the work area. The distance to the level below was approximately 9 feet.
