29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: Second floor Patron's Lounge, On or about June 10, 2014, and at times prior thereto, employees were exposed to a fall greater than 6 feet in height to the concrete below, from a scaffold that did not have the wheels in a locked position, while accessing the scaffold to prep the door frames and ceiling to paint.

29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.
