Outdoor Diesel Fuel Tank, On or about June 17, 2014 and at times prior thereto, employees were exposed to the hazard of falls of approximately 7 feet 2 inches in height to the ground level below, when standing on the top stair step of the industrial stairway measuring 11 inches in width, while gauging the fuel level of the tank.
