29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft; a) On or about 6/18/14 in the spice room off of the meat mixing room; employer failed to provide a level of protection provided by a personal lockout when it allowed two employees, one cleaning the sausage stuffer and the other cleaning the chopper, to use one lock on a hasp which closed the electrical panel that contained the two "opened" circuit breakers for this equipment exposing the employee without the lock to a caught between hazards of the unexpected start up of the equipment. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained. a) On or about 6/18/14 in the spice room off of the meat mixing room; employer failed to require each employee involved in cleaning the sausage stuffer and the chopper, to use one lock on each of the energy isolation devices for this equipment exposing the employee without the lock to a caught between hazards of the unexpected start up of the equipment. ABATEMENT CERTIFICATION REQUIRED
