29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). On or about June 09, 2014, and at prior times, at the BP Amoco Chemical Company in Texas City, TX, the employer did not ensure that all energy isolating devices was capable of isolating all energy sources to T-501 and the spool piece in the RV548 outlet piping, while performing maintenance in the PX1 Unit.

29 CFR 1910.147(d)(5)(ii): If there is a possibility of reaccumulation of stored energy to a hazardous level, the employer shall continue verification of isolation until the servicing or maintenance is completed, or until the possibility of such accumulation no longer exists. On or about June 09, 2014, and at prior times, at the BP Amoco Chemical Company in Texas City, TX, the employer did not ensure that there was a continued verification of isolation when working on the RV548 outlet piping while performing maintenance on the PX1 Unit.
