29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      The employer is failing to protect employees from potential fall hazards associated with re-roofing an existing residential home where the ground to eave is greater than 6 feet and the roof pitch is more than 4:12.  The most recent example of this was found on the job site located at 12938 Eagle Run Dr. Omaha, NE 68164 where the employer has employees using rock climbing gear in lieu of ANSI approved fall arrest harnesses, thus failing to provide the employees with adequate fall protection in the event that an employee would fall off the roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    The employer is failing to provide training to employees on the potential fall hazards associated with re-roofing an existing residential home where the ground to eave is greater than 6 feet and the roof pitch is more than 4:12.  The most recent example of this was found on the job site located at 12938 Eagle Run Dr. Omaha, NE 68164 where the employer has improperly trained employees using rock climbing gear in lieu of ANSI approved fall arrest harnesses, thus exposing the employees to a potential fall as a result of their inability to recognize the potential hazards of using improper equipment.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      The employer is failing to protect employees from potential fall hazards associated with using ladders as a means of access and egress to and from elevated work surfaces where the ground to eave is greater than 6 feet and the roof pitch is more than 4:12.  The most recent example of this was found on the job site located at 12938 Eagle Run Dr. Omaha, NE 68164 where the employer has employees using two aluminum and one fiberglass ladder as a means of access to the roof of a new residential home without extending the ladder a minimum of three feet over the upper landing surface.

The most recent example of this was found on the job site located at 12938 Eagle Run Dr. Omaha, NE 68164 where the employer has inadequately trained employees using an aluminum fold up ladder as a means of access to the roof of a new residential home without extending the ladder a minimum of three feet over the upper landing surface.
