The most recent example of this was found on the job site located at 933 N. 150th Street, Omaha, NE 68147 where the employer has employees re-roofing an existing residential home without the aid of fall protection thus exposing those employees to potential fall hazards greater than 8.

The most recent example of this was found on the job site located at 933 N. 150th Street, Omaha, NE 68147 where the employer has untrained employees re-roofing the house without using fall protection thus failing to recognize the potential hazard of working on a roof over 8 above the ground and a roof pitch greater than 4:12.
