On or about June 18, 2014, in an 800S Skypower aerial lift, two employees performing roofing operations were not protected from a 46-foot fall hazard by use of a fall protection system.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a. On or about June 18, 2014, on a steep pitched roof, an employee performing roofing operations was not protected from a 46-foot fall hazard by use of a fall protection system.   The WKL Roofing, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 622678, citation number 01, item number 002 and was affirmed as a final order on November 13, 2012, with respect to a workplace located at 2325 S. Ridgewood Avenue, Edgewater, Florida 32132.
