(a) Jobsite, 2805 Freeport Road, Harmar Township, PA:  On or about 06/18/14 an employee working on the mezzanine section of the building engaged in precast concrete operations was not protected from falling approximately 20 feet to the ground below.  The employee was on the mezzanine section scraping and sweeping and was not protected from falling by guardrail systems, safety nets or personal fall arrest systems.
