The employer did not ensure that each chute or hatchway floor opening was guarded by either a hinged floor opening or standard strength and construction equipped with standard railings as to leave only one exposed side or a removable railing with a toeboard on not more than two sides of the opening and fixed standard railings on all other exposed sides. This violation occurred at the right side bagger machine in the shipping building. b. The employer did not ensure that where a hinged floor opening was installed on a hatchway or chute floor opening, there was a standard railing and/or removable railing with toeboards installed. This violation occurred at the left side bagger machine in the shipping building.
