29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems: Employees were exposed to a fall hazard over 25 feet while installing riveting caps and metal roofing and employees were not protected by guardrail systems, a safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: Employees were exposed to a fall hazard up to 25 feet while working out of an aerial lift installing riveting caps and metal roofing and the employees did not wear a body belt or lanyard attached to the basket. Steel Pro Structures, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 312888241, citation number 1, item number 2 and was affirmed as a final order on August 8, 2009, with respect to a workplace located at 1700 Recreation Drive Exit #93, Springfield Illinois, 62711.
