A. The Black Pearl Builders LLC worksite located at 921 S. Main, Findlay, Ohio: On or about June 17, 2014, the employer failed to ensure an employee did not stand on the guardrails of the aerial lift to perform painting activities on the two story residential structure. The employee was exposed to a potential fall due to the weak working position.

Main, Findlay, Ohio: On or about June 17, 2014, the employer did not ensure an employee was protected from fall hazards while working in the aerial lift. The exposed employee was performing painting activities with no harness and lanyard.
