29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket and were sitting or climbing on the edge of the baskets guardrails or using planks, ladders, or other devices for a work position.    A. The Black Pearl Builders LLC worksite located at 921 S. Main, Findlay, Ohio: On or about June 17, 2014, the  employer failed to ensure an employee did not stand on the guardrails of the aerial lift to perform painting activities on the two story residential structure. The employee was exposed to a potential fall due to the weak working position.

29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift:    A. The Black Pearl Builders LLC worksite located at 921 S. Main, Findlay, Ohio: On or about June 17, 2014, the employer did not ensure an employee was protected from fall hazards while working in the aerial lift. The exposed employee was performing painting activities with no harness and lanyard.
